Dr Sue Pedersen speaks about the clinical trials done on portion control

The Diet Plate trial, which I conducted at the University of Calgary, was the first trial in the world to study a portion control tool to help people lose weight... and it worked! Let me tell you the story of how the trial came to be.

Knowing how oversized the portions available to us are, and knowing how much they have grown in recent years, I went in search of a portion control plate that I thought contained appropriate portions, would be easy to use, and would be appealing enough that it could join a home dinnerware collection! With these criteria in mind, I reviewed the portion control tools that were available, and I settled on The Diet Plate ® ™ to study, because it fit all of these criteria nicely.


I contacted Kay Illingworth, the CEO of The Diet Plate ® ™ in Britain, and asked her if she would donate plates that I could use in my study. Kay is the inventor of these plates, which she designed to combat her own struggle with overweight. She tells a pretty funny story about the day the plates came to be: she got incredibly frustrated with her inability to control portions, which led her to take her entire kitchen plate set out and to use a marker to draw all sorts of various test portions out on them. Little did she know, it was not a water soluble pen! However, it was worth the ‘investment’: with her final portion determination created, she proceeded to lose weight using the plate. Same for her mother. Word soon spread about these successes... and the rest is history!

Suffice it to say, when I contacted Kay about my study, she was more than happy to oblige...and soon thereafter I had my clinical trial underway.

In this study, we looked at the effectiveness of The Diet Plate to help obese people with Type 2 Diabetes lose weight. Half of the study participants received The Diet Plate and The Diet Bowl to use for a 6 month period, and we compared their weight loss to the weight loss of a control group, who did not receive the plate products. We found that the group using the plate were almost 4 times more likely to lose 5% body weight, compared to controls! This ‘magic number’ of 5% is very important, because studies have shown that if an overweight individual can drop 5% of their body weight, they decrease their risk of getting diseases related to being overweight, and also decrease their risk of dying from these diseases (bluntly put). When we looked at people who were committed to using the plate on 80% or more of study days, we found that they were SIX TIMES more likely to lose that 5% of body weight compared to people who were not using the plate system.

Remembering that all of the participants in the study had type 2 diabetes, the other important thing that we found was that people using the plate system were over twice as likely to require a decrease in their diabetes medication over the 6 month trial compared to people not using the plates. Conversely, people who were not using the plates were over twice as likely to need an increase in their diabetes medication, compared to the plate group! The tendency of T2DM is to get worse with time, as insulin resistance worsens, and the ability of the pancreas to combat this resistance wears with time. So, we were seeing the typical progress of diabetes in the control group, but the plate system was able to put a halt to the progress of diabetes in many of these people, and even to reverse it and improve it in many! This is a powerful testament to the efficacy of portion control.

Given that the entire group that we studied were Type 2 Diabetics, our results are even more impressive, because this is a group of people in whom it is notoriously difficult to achieve weight loss. Much of this has to do with the medications that they are taking, in that most medications that treat diabetes cause weight gain as a side effect. In our study, 94 out of 130 people were taking medication for their diabetes that causes weight gain as a side effect, and STILL we were able to show that this plate system was a success! Individuals who are not diabetic are as likely, if not more likely, to have success with this plate system.

This plate system makes sense because they are simple to use, and because they give you the freedom to choose what you would like to eat. Of course, I always advocate for healthy food choices, but this system does allow you the freedom to choose what you would like to have – you just have to portion it according to the boundaries on the plate!

There are many, many different types of portion plates and similar tools out there. ‘Would a different portion control tool work just as well?’ you might be wondering. Well, at this point in time, the Diet Plate ® ™ is the only one that has been studied and proven to work in a randomized, clinical trial fashion (the ‘gold standard’ type of trial). I am certain that a number of the other tools out there would work, but they need to be studied in a similar fashion for the evidence to be there to recommend them.

"No two people are the same shape. We might have the same height and weight, but just how that weight is made up can make all the difference. Although it’s a popular get-out clause for being overweight some people are genuinely “big-boned” with thicker(heavier) bones and a wider set skeleton. A very physically active person might have developed significant muscle mass on their arms and legs and have a lean shape but be the same weight as another who is inactive and who has a large collection of fat around their middle. Women tend to collect excess fat around their hips and thighs, the so called “pear-shape” whereas men often carry it around their waist, the “beer-belly” or “apple-shape”. The best way to work out if you have too much fat is to measure around your waist. If it’s more than 90cm (35 inches) in a woman, or 102cm (40 inches) in a man you’re at increased risk of overweight associated medical problems. There's never a better time to start losing weight than right now."

The Diet Plate and losing weight

Losing weight is never easy, it is wrapped up in emotional issues, a general love of food, sedentary lifestyles, age and with some people a couldn’t care less approach to life. The latter normally enjoying youth and tighter waistlines!

Diabetes solution served up on a plate

There is a simple solution to the predicted explosion of diabetes…and it can be served up on a plate, according to a recent study published in the Annals of Internal Medicine. And what’s more the ‘Diet Plate’ will help reverse the obesity epidemic as well.
A six-month study from Calgary University in Canada split 130 people with type 1 and type 2 diabetes into two equal groups. Half were given only dietary advice, the other half The Diet Plate system – which includes a portion-controlled dinner plate and breakfast bowl. Results showed those using The Diet Plate were more likely to lose at least 5% of their body weight and decreased their reliance on diabetes medication. “This amount of weight loss is significant” said Dr Ian Campbell, medical adviser to The Diet Plate. “The most important treatment for people with diabetes is to improve their diet, and lose weight, and yet they find it harder to lose weight than non-diabetics. Using The Diet Plate made weight loss more likely, improved diabetes control and a decrease in prescribed medication.”
The study author, Dr Sue Pederson said “our results are comparable to similar studies undertaken using expensive weight loss drugs, but with no need for close medical supervision or risk of unpleasant side effects”.

Men and Toxic Waist

What is it about men that makes them so reluctant to take their health seriously? When it comes to obesity, despite the fact that as many men are overweight as women, only 1% of those attending weight loss clubs, and only one in five attending medical clinics, are male. Unlike women who are ready at an early age to openly talk about their weight, men are more likely to bottle it up until after they’ve suffered one of the health consequences, like angina or diabetes. And yet all men are vulnerable to becoming overweight. Even the thinnest, most active among us can experience rapid weight gain after a major life event, job change, or sports injury. But for most of us, a sustained but mere 100 calories a day excess is enough to invite the creeping shadow of weight gain, with one or two pounds of gain in a year that eventually shows itself as an ever increasing circle of fat around our waist.

No surprise then that two thirds of men should consider themselves to be overweight and at risk of disease. Some might even view being fat as an occupational hazard of a sedentary lifestyle and working life. Spending one’s working week sitting at an automated machine, a desk, or at meetings, grabbing a meal when you can, and working long hours with little scope for exercise would create a considerable health challenge for any bloke. It should also create a better understanding among us of how obesity is in the majority of cases not simply due to greed or laziness, but due in large part to modern life.

Men tend to carry our excess fat very conspicuously around our waist. Unfortunately this beer-gut or “apple-shaped” obesity is also the most dangerous, being responsible for the development of raised cholesterol, high blood sugar, and high blood pressure. The good news is that although a man’s waist circumference is a better indicator of heart disease risk, it is also a greater motivator towards weight reduction. Men are more likely to get excited about decreasing their belt size than shedding the odd pound or two. One centimetre of waist loss usually means 1 kg, or 2 pounds, of fat loss, and brings with it the same health benefits.

If we are to make any headway into tackling this modern epidemic it’s time we stopped blaming ourselves. Of course responsibility for health always begins with ourselves, but a recognition of the part our lifestyle plays in making us more prone to being overweight is important if we are going to be able to do anything long term about it. Overweight affects two thirds of all men. Let’s stop looking at them, and start looking at ourselves. After all, how can we take a speck of dust from a friend’s eye, when we still have a plank in our own!

Obesity: whose responsibility?

Few can have failed to notice that being fat is now at the top of the medical, and political agendas. Twenty-five percent of UK adults are now classified as obese, with another forty-two percent being overweight. Doctors have been getting excited about this for years, government has been slowly catching on, and now, finally, the public is starting to take notice themselves. A conversational subject once confined to women’s diet clubs is now being talked about in the workplace, the pub, the classroom, and around the dinner table.

Carrying too much weight doesn’t feel good. It might not look good either. And it certainly doesn’t improve your performance at work, in the field, or the office, or at home. An obese man has four times the risk of heart disease; an obese woman a twelve-fold risk of type 2 diabetes. The biggest preventable cause of cancer, after smoking, is obesity, and if you die of an obesity related disease you’re dying on average nine years too early. Obesity costs the NHS around £3.5 billion annually.

So what can be done about it? The 2007 Foresight Report called for more action to prevent, and treat obesity, at schools, at work, and at home. They promised a number of innovative measures to reduce the likelihood of our population continuing to gain weight. This year has seen the launch of the exciting new public health campaign from the Department of Health, called Change4Life, encouraging us all to start making those simple changes that can make all the difference. As the maxim goes, if you want a job done well, do it yourself. While national programmes are constructed to help us all, it still falls to us as individuals to try and help ourselves, to rise up to the challenge of our personal responsibility. It’s said that an apple a day keeps the doctor away; a little weight loss can achieve the same result. Being active at every opportunity, reducing alcohol, fat and sugar intake, and reducing our food portion sizes can go a long way to addressing the problem. There has never been a better time to start a healthier lifestyle, and reap a lifetime of benefit. More than ever, we owe it to ourselves. Come on, let’s do it! Start making those changes now, and keep those Changes4Life.
